1 : Every Declaration of conformity should be identified for easy reference
2 : Identification of the manufacturer or where appropriate of the Authorised representative. For larger companies it may be necessary to specify operation groups or departments.
3 : Identification and description of the apparatus to which the declaration od conformity refers. The apparatus should be unequivocally described so that the declaration may be related to the apparatus. For mass-products it is not necessary to give individual serial numbers.
4 : Reference to the specifications under which conformity is declared and where appropriate, to the national measures implemented to ensure the conformity of the apparatus with the provisions of the Directive (or Directives). Where appropriate, reference must be given to the EC type-examination certificate issued by a notified body.
5 : Text should appear there only if additional information is given. It should be tailored to the type of information.
6 : Identification of the signatory empowered to bind the manufacturer or his representative.
