Unless you were producing or administering only small volumes of a process's outputs, checking every action and every outcome would be a hugely laborious, time-consuming and expensive way of going about things.

Imagine having to check every bottle top if you were producing 10,000 an hour! It's too late then, anyway. The process requires Quality Assurance to build confidence and predictability.
